Onley, Va. — With an estimated 13% of Northampton County residents over age 65 living with Alzheimer’s—and many more experiencing other forms of memory loss—access to specialized memory care is a growing concern on Virginia’s Eastern Shore.
To help bridge that gap, Riverside Health System is bringing its Geriatric Assessment Clinic to Riverside Eastern Shore Family Medicine on August 28 and 29, offering expert evaluations, memory screenings, and individualized care planning for older adults in the community.
The clinic, typically based at the Martha W. Goodson Center in Williamsburg, is led by a board-certified geriatrician and supported by an interdisciplinary team. The mobile visit represents a major step in expanding geriatric and memory care services to a rural region where geographic barriers can make accessing specialized care difficult.
Bringing this clinic to the Eastern Shore reflects Riverside’s commitment to delivering expert, compassionate care—where and when it’s needed most. The goal is to meet older adults and their caregivers where they are, providing tools to support independence, well-being, and peace of mind.
Services provided at the clinic include memory screenings to detect early signs of dementia, reviews of fall risks and nutritional issues, medication reconciliation to prevent harmful interactions, and assessments of mood, behavior, and safety concerns. Based on the evaluation, patients may also be referred for lab tests, CT scans, or neuropsychological assessments. Follow-up care may include enrollment in Riverside’s Memory Care Navigation program, which supports patients and caregivers through ongoing challenges related to memory loss.
